COMPANY: Adena Hospice HOW YOU'LL MAKE A DIFFERENCE: As a Hospice Registered Nurse Case Manager (RNCM) you will be able to help patients and their families navigate the most intimate and difficult time of one’s life. Using hospice best-practice care delivery approaches, you will have all the tools and resources needed to ensure an excellent end-of-life experience for your patients. WHAT YOU WILL LOVE DOING: Symptom management: Through a thorough assessment you will work closely with the patient’s attending physician and the hospice Medical Director to ensure every patient’s symptoms are well managed. Plan of Care: Evaluate the treatment and medical condition of the patient according to the plan of care. Individualize the plan of care and adjust as needs change. Continuity of Care: Manage interdisciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges. Family Educator/Advocate: Counsel patient and family on the disease/progression and incorporate patient and family in development of plan of care. Listen to patient and family members. Documentation: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines. Operations: Oversee and supervise total care of patient provided by nurse aides and LPNs. Attend in-service training and mandatory agency meetings. REQUIREMENTS: Registered Nurse with current license in the state of employment. Minimum one-year experience as an RN in an acute care setting is preferred. Hospice experience is desired. Meridian St., Indianapolis, IN 46204 Schedule: 8:00am to 4:30pm Monday - Friday HOW YOU'LL MAKE A DIFFERENCE: As a Hospice Program Assistant, the work you do every day makes a difference in the lives of our patients by assisting the field staff in providing exceptional care by scheduling visits to meet client care needs effectively and efficiently. HOW YOU'LL WORK: You'll assign new patients to clinicians according to scheduling protocols. You will ensure that all patients' and field staff inquiries or concerns are addressed courteously and promptly adding to the overall outstanding patient experience. MAJOR AREAS OF RESPONSIBILITY: <ul><li>Coordinate day-to-day schedules for field staff. </li><li>Address missed, declines, unverified, and reassigned visits with field staff. Manage daily workflow in company database and process daily and weekly report. </li><li>Respond promptly and courteously to all patients' inquiries or problems. </li><li>Collaborate with director/supervisor regarding client concerns or complaints. </li><li>Understand and practice agency policies and procedures and follow Medicare guidelines. </li></ul>HARD & SOFT SKILLS:<ul><li>Compassionate communicator with a positive attitude. </li><li>Patience is a virtue when working with patients, families, physicians, and coworkers. </li><li>Attention to detail is critical, as is being observant and following directions. </li><li>Critical thinker and the ability to problem solve clinical needs. </li><li>Technical skills to include Microsoft products and EMR systems. </li></ul>REQUIREMENTS:<ul><li>High-School graduate or equivalent </li><li>Associates Degree preferred </li><li>At least one year of experience in the Health Care industry and experience in home care/hospice is desirable </li><li>Proficiency in medical terminology </li><li>Capable of all physical demands </li></ul> We are proud to be part of the Alternate Solutions Health Network family. #INDRED2 We'll help you put your passion for patient care to work.
