The Community Partners Case Manager works independently in the field providing home-based and community-based services to families referred to Iris from the Department of Child Services, self-referrals, and other community sources. While supervision and team support are available, the Case Manager primarily works solo throughout the day, managing their schedule, prioritizing needs, and ensuring that families receive consistent, high-quality support to promote safety, stability, and well-being.
