The LCSW Care Manager is responsible for managing care for high-risk members with chronic behavioral and health conditions, collaborating with members, caregivers, physicians, and the healthcare team to ensure timely access to necessary care, continuity across all settings, shared decision-making, and connections to supportive services and community resources. Current Licensed Master Social Worker (LMSW) or under supervision for Licensed Clinical Social Work (LCSW) or LCSW from the Oklahoma State Board of Licensed Social Workers.
