This role supports participant safety, aging in place, and social determinants of health (SDOH) interventions; oversees grievances, service delivery requests, and incident investigations; guides staff through complex cases including housing instability, facility placement, eligibility barriers, and transitions of care; and serves as a key liaison among the IDT, Center Triad (Center Medical Director, Center Director, and Director of Nursing), and community partners. The Social Services Supervisor is accountable for both team and individual performance outcomes, including: Regulatory and documentation compliance (timely assessments, care plans, SDRs, grievances, incident documentation).
