The Physical Therapist will provide skilled therapy services to patients in their homes, including evaluations, treatment planning, therapeutic interventions, patient and caregiver education, and ongoing assessment of patient progress. Educate patients, families, and caregivers on exercises, mobility techniques, safety, and home programs.
