Coordinates the planning and organization of multidisciplinary clinic activities, including development of the weekly interdisciplinary schedule, coordination of provider and patient appointments, and communication among physicians, nursing, social work, nutrition, and other specialty services. Obtains and reviews medical records for new and established patients, preload relevant histories, complete medication reconciliation, confirm appointments, and ensure required patient information and assessments are available prior to clinic visits.
