Care Management Workflow Ownership: Manages assigned practice workflows-such as pre-visit planning, workflow dashboard management, charge entry, claim management, and clinical inbox management-and executes specialized care workflows including Transitional Care Management (TCM), Chronic Care Management (CCM), and Remote Patient Monitoring (RPM) when active in the practice. ESSENTIAL RESPONSIBILITIES / DUTIES: Patient Intake and Rooming: Rooms patients and prepares them for provider visits by accurately measuring and recording vital signs (including blood pressure, temperature, pulse, pulse oximetry, weight, height, and respiratory rate) and documenting patient history, chief complaints, pharmacy information, and prescription lists directly into the Electronic Medical Record (EMR) system.
