Conduct comprehensive palliative care assessments and follow-up visits• Manage patients with serious illness and complex chronic conditions• Facilitate goals-of-care conversations and advance care planning discussions• Develop and implement individualized care plans• Collaborate with physicians, specialists, hospitals, skilled nursing facilities, and community providers• Provide symptom management and patient/family education• Document patient encounters accurately and in a timely manner• Participate in interdisciplinary team meetings and care coordination activities. • Active New Jersey Nurse Practitioner license• National board certification and DEA registration• Minimum 2 years of Nurse Practitioner experience preferred• Experience in palliative care, hospice, geriatrics, oncology, family medicine, internal medicine, or chronic disease management preferred• Strong communication and interpersonal skills• Ability to work independently while collaborating within a multidisciplinary team.
