In this role, you will lead a team of field-based Community Health Advocates who conduct home visits, accompany members to medical appointments, perform social needs assessments, connect members to community resources, and provide hands-on support to improve access to care. The Community Health Advocate team is responsible for improving health outcomes and reducing barriers to care for Blue Shield Promise Health Plan members through field-based outreach, care navigation, community engagement, and support services that address social drivers of health.
