Job Title: Master of Social Work (MSW) – Travel ContractLocation: Napa, CA Contract Length: 13 Weeks Shift: Day Shift | 5x8s | 8:00 AM – 4:30 PMWe are seeking a compassionate and experienced Master of Social Work (MSW) professional for a travel assignment in Napa, California. The ideal candidate will provide psychosocial support, discharge planning, care coordination, and advocacy services while collaborating closely with interdisciplinary healthcare teams.Responsibilities<ul><li>Conduct comprehensive psychosocial assessments of patients and families</li><li>Develop and implement individualized care and discharge plans</li><li>Provide crisis intervention, counseling, and emotional support services</li><li>Assist patients with accessing community resources, financial assistance programs, and support services</li><li>Collaborate with physicians, nurses, case managers, and healthcare professionals to coordinate patient care</li><li>Advocate for patients’ rights and promote patient-centered care</li><li>Facilitate discharge planning to ensure safe and effective transitions of care</li><li>Identify barriers to treatment and develop strategies to address patient needs</li><li>Maintain accurate and timely documentation in accordance with facility and regulatory standards</li><li>Participate in multidisciplinary team meetings and case reviews</li><li>Educate patients and families regarding available healthcare and community resources</li><li>Ensure compliance with hospital policies, state regulations, and ethical standards of social work practice</li></ul>Requirements<ul><li>Master’s Degree in Social Work (MSW) from an accredited program required</li><li>Current California Social Work licensure or eligibility required</li><li>Previous experience in a healthcare, hospital, or acute care setting preferred</li><li>Strong knowledge of discharge planning, care coordination, and community resource management</li><li>Excellent communication, interpersonal, and counseling skills</li><li>Ability to work effectively in a multidisciplinary healthcare environment</li><li>Strong assessment, critical thinking, and problem-solving abilities</li><li>Experience in acute care, medical social work, or case management preferred</li><li>Familiarity with EMR systems preferred</li><li>Knowledge of Medicare, Medicaid, and healthcare regulations preferred</li><li>Experience working with diverse patient populations preferred</li></ul>For more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.About Navitas Healthcare, LLCIt is a Joint Commission Certified / WBENC and one of the fastest-growing healthcare staffing firms in the US providing Medical, Clinical and Non-Clinical services to numerous hospitals.
