This is a field-based relationship role: you will be out in the community every day, building trust with physicians, hospital discharge teams, skilled nursing facilities, and case managers — and serving as the bridge between families navigating an incredibly difficult time and the care they deserve. The right candidate will recognize themselves in most of the following: You build trust quickly — in a hospital hallway, a SNF conference room, or a family's living room, you know how to show up and earn credibility fast.
