Housing Case Manager (Family Advocate III) Office Location: 1848 East Perry Street, Port Clinton, OH 43452 Service Area: This position provides services throughout Erie, Huron, Ottawa, Sandusky, Seneca, Richland, and Wood counties. Recruit, screen, assess, establish goals, and make referrals related to employment, housing, health care including substance abuse and/or alcohol abuse, economic self-sufficiency, and interpersonal relationships to assist participants and their families meet their basic needs.
