The Young Adult Navigator will provide supportive services focused on individualized service planning, benefit entitlement support and assistance, and supportive case management for 87 young adult individuals aged 18-25 and 43 families with children headed by young adults aged 18-25. The Young Adult Navigator will: Conduct initial intake and comprehensive needs assessments for tenants and tenants’ families’ needs, focusing on the whole family as well as each individual member needs.
