Support convenings of healthcare providers, lactation support providers, home visiting organizations, doula organizations, medical respite programs, housing providers, food/nutrition providers, community health workers, managed care organizations, state agencies, and other partners to advance systems improvements and alignment. The primary goal of the Social Health Care Network is to build the network of providers that will deliver health related social needs (HRSN) services under the Illinois Medicaid 1115 waiver, community health worker (CHW) services, and new maternal health services (e.g., doulas, lactation consultants) and provide training, technical assistance and capacity building funding.
