Under the direction of the primary care operations team, the Primary Care Patient Care MA works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. Working the Encounter Guide including helping to close any open care gaps such as fall risk, depression screening, breast cancer screening, colorectal cancer screening, and diabetic eye exam.
