Health Home Care Manager Education & Qualifications:A bachelor's degree in any of the following: child & family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation, recreational therapy, rehabilitation, social work, sociology, or speech and hearing, or other human services field, orNYS licensure and current registration as a Registered Nurse and a bachelor's degree; ORNYS Teachers Certificate for which a bachelor's degree is required; ORBachelor's level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses; ORCredentialed Alcoholism and Substance Abuse Counselor (CASAC).Two years' experience (a Master's degree in a related field may substitute for up to one year of experience) either: Providing direct services to persons with serious mental illness, developmental disabilities, alcohol or substance abuse; OR Linking persons who have serious mental illness, developmental disabilities, alcohol or substance abuse to a broad range of services essential to successful living in a community setting (e.g. Health Home Care Manager Job Description:Established in 1948, the Mission of ASCEND Mental Wellness is to encourage and assist individuals in discovering pathways to their optimal mental health and wellness, while supporting hope and awareness of well-being in our community.
