Conducts psychosocial assessments, develops and implements intervention and treatment plans, facilitates access to resources and services, and collaborates with interdisciplinary teams to support optimal patient outcomes. Responsibilities may vary based on assigned department, specialty area, or clinical setting, including but not limited to inpatient, outpatient, behavioral health, psychiatry, emergency, specialty care, and care management services.
