Maintain assigned caseload; Providing direct care to residents – including assessments, individual counseling, discharge and aftercare planning, treatment plans and updates, weekly progress notes, daily groups facilitation, group note completion, and completion of bio-psychosocial assessments; Making referrals when appropriate; Responsible for each assigned residents’ medical records. All records are to be professional, complete and thorough with entries made in a timely manner; Responsible for linking clients with aftercare services prior to discharge and aiding the resident and his/her family access services when appropriate; Strengthen current and create new partnerships within the community to ensure that the clients develop the skills needed to sustain sobriety and/or housing; Review patients'/family rights and responsibilities, emergency safety plans and grievance policy.
