Provides direct service to members through assessment, case management, group counseling, job support, crisis intervention, transition coordination, and psycho-social rehabilitation services in the community and at Thresholds locations. The Case Manager coordinates treatment, develops and implements individualized service plans, facilitates services - like school, medical providers, family, etc.
