Maintain effective communication with health care team members related to assessment findings, discharge planning needs and provider orders needed to arrange Homecare, Durable Medical Equipment, Transportation, Skilled Nursing or Acute Rehab Facility placement, Substance Abuse Treatment and outpatient follow-up. Provide psychosocial assessments, diagnosis, and treatment, as well as consultation about patients and families to assist them and the health care team in coping with patients hospitalization, illness, diagnosis, treatment, and/or life situation, including emotional, mental, and substance abuse disorders.
