This role works collaboratively with internal departments, community partners, hospitals, primary care providers, social service agencies, and leadership to strengthen care transitions, address social determinants of health, and support complex patient populations through innovative care models. The clinical manager will be the primary point of contact for state-administered, federally funded programs, namely Children’s Integrated Services (CIS) and Maternal, Infant, and Early Childhood Home Visiting (MIECHV) Program.
