The Lead Care Manager serves as the client’s primary point of contact and works with all their providers – such as doctors, specialists, pharmacists, social service providers, and others – to make sure everyone is in agreement about the client’s needs and care. These services include but are not limited to linkage to mental health services, medical care, educational services, pre-vocational services, vocational services, rehabilitative services, substance use treatment, housing and other community supports.
