This professional nursing role requires advanced clinical knowledge, strong communication and organizational skills, and the ability to coordinate care collaboratively with Cardiologist, physicians, fellows, advanced practice providers, nurses, ancillary staff, referral sources, and other healthcare partners. The RN works collaboratively with the patient, family, and healthcare team to develop and implement individualized plans of care and supports patients in effective self-management, with an emphasis on symptom recognition, disease stability, quality of life, and prevention of avoidable hospitalizations and readmissions.
