Order and interpret diagnostic tests, prescribe pharmacologic and non-pharmacologic treatments, and provide point of care testing, vaccinations, and medication administrations for cardiovascular patients. Work with multidisciplinary teams including behavioral health, social work, nutrition, pharmacy, home care, physical rehabilitation, hospice and other specialists as necessary to provide/coordinate care across the continuum.
