Identify and pursue new referral sources, including hospitals, skilled nursing and rehab facilities, assisted living communities, physician practices, case managers, social workers, and community organizations. As we continue to grow throughout Pennsylvania, we are looking for a motivated, relationship-driven professional to help expand our community presence, develop referral partnerships, and connect more individuals and families with the care they need.
