Assessments may include, but are not limited to social history, PHQ (patient health questionnaire), Cage Questionnaire (“cut, annoyed, guilty, eye” – identify problems with alcohol), Tobacco Usage, GPRA (“government performance & results modernization” – collect, improve, report outcomes), Social Determinants, etc. This position is responsible for comprehensive intake procedures including patient registration, screening, assessment, and coordination with the healthcare team to ensure patients receive appropriate and timely care.
