GENERAL DESCRIPTION: The Connection 2 Care (C2C) Program Manager oversees the outreach, counseling, navigational, and case management services to recently released prisoners including those affected by chronic diseases, substance use (SU) and/or mental health issues who present at or are referred to the Connection 2 Care Program (C2C) at MyCare Health Center.
