Working in partnership with Social Work professionals, you'll coordinate comprehensive discharge planning and resource management for approximately 40 patients with diverse medical needs including diabetes, heart failure, COPD, respiratory conditions, post-surgical care, and more. This position is ideal for an experienced case manager who thrives on independence, enjoys variety, and is passionate about facilitating positive patient outcomes across the care continuum.
