Under the supervision of the Ryan White Case Management Coordinator, the Medical Case Manager coordinates the medical care of clients living with HIV/AIDS, in conjunction with the Medical Management Clinic at LCHD/CHC and works with clients to develop a supportive relationship while facilitating access and use of available services. With an associate's degree in a human services field, four years of work experience working with the public, preferably in a related field (public health, health education, social work, psychology, sociology, early child development, case management, etc.).
