Responsible for providing intensive, wrap-around case management services for Williams/Colbert Class Members to facilitate a safe, smooth, and expedient transition from structured facility and into independent living in the community. Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest and fair manner.
