Required:• Current Michigan Registered Nurse (RN) license in good standing• One year of nursing experience• Valid Michigan driver's license with acceptable driving record• Ability to travel throughout Clinton, Eaton, and Ingham CountiesCandidates with experience in hospital nursing, home health, hospice, discharge planning, case management, care coordination, public health, or related healthcare settings are encouraged to apply. Arranges, coordinates, and monitors the services according to the person-centered plan, including those covered through the Medicaid Waiver and Bureau of Aging, Community Living, and Supports (ACLS Bureau).Monitor the cost of Medicaid Waiver services and other formal services to maximize resources and stay within program parameters and goals.
