POSITION DUTIES – Assesses patients' physical, emotional, and psychosocial needs; develops and coordinates individualized end-of-life care plans; manages symptoms and pain control; educates patients and families; collaborates with interdisciplinary hospice team members; monitors care outcomes; and provides ongoing support to ensure comfort, dignity, and quality of life in the home setting.
