Develop and maintain working relationships with community agencies to assist with plan management and goal achievement, including wraparound services, inpatient treatment centers, outpatient treatment groups, legal teams, government agencies, managed care, referral sources, educational systems and probation departments. The primary function of the Case Manager is to oversee all aspects of case management, including interviewing families, assessing needs, determining risk categories, making referrals, implementing appropriate plans of care and planning follow-up services.
