Work consists of: case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services, as well as new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall. We're looking for an experienced RN Case Manager interested in building a long-term employment relationship with our agency by consistently accepting RNCM and recertification visits as needs arise, and providing in-office State Survey support as needed (1-2x/year) in our Indianapolis Office (8580 Cedar Place Dr, Indianapolis, IN 46240), under the guidance of our Indiana Administrator.
