YH6-0014, a Case Manager must be an Arizona licensed registered nurse in good standing OR possess a degree in social work OR possess a bachelor''s degree in psychology, special education, or counseling AND have at least one (1) year experience providing case management services to persons who are elderly and/or persons with physical or developmental disabilities and/or members determined to have a Serious Mental Illness (SMI) OR have a minimum of two (2) years' experience providing case management services to persons who are elderly and/or persons with physical or developmental disabilities and/or members determined to have a Serious Mental Illness (SMI). Conducts field visits per agency policy requirements at various locations: family homes, group home, other residential placements, psychiatric hospitals, schools, day programs, work programs, hospitals, etc. to develop relationships with the child/adult and families.
