Job Description Summary: With the support of and in partnership with the C3 team, the Case Manager performs a variety of tasks to assist the most vulnerable and chronically homeless individuals and is responsible for identifying, engaging, and providing support to vulnerable homeless program participants, including providing aftercare and follow up support, once housed.
