Job Summary: The Home Health/Hospice RN will provide case management and skilled nursing services, supervise CNAs and PRN nurses, and coordinate care among home care disciplines, physicians, non-physician practitioners (NPP), and patients. members, caregivers, physicians and medical center staff, discharge planners, other hospital personnel, all members of the health care team, and support staff.
