This Care Coordinator works directly within one of our partnered-dialysis clinics to engage patients in person, coordinate care plans in collaboration with Ennoble providers and facility staff, schedule and organize provider visits, and ensure timely follow-up and clear communication with patients, families, and the clinical team. Ennoble Care offers a variety of programs including, remote patient monitoring, behavioral health management, and chronic care management, to ensure that our patients receive the highest quality of care by a team they know and trust.
