The Registered Nurse (RN) provides general and skilled nursing care to participants, executes interventions in accordance with the plan of care, implements primary care orders, evaluates participant complaints in person and/or by telephone, develops appropriate plan of care, provides participant, caregiver and family education and communicates participant’s condition to primary caregivers, physician and care team. Senior Community Care of Kentucky PACE (Northern) provides healthcare and supportive services for individuals age 55 and older, helping them live safely, comfortably, and independently in their own homes and communities.
