You will collaborate with physicians, RN Case Managers, social workers, chaplains, hospice aides, and other members of the interdisciplinary team to help ensure comfort, dignity, and continuity of care. Notify the physician, Medical Director, RN Case Manager, or appropriate clinical leader of significant changes, uncontrolled symptoms, safety concerns, or other urgent needs.
