Works closely in collaboration with department heads, physicians, ancillary providers, Sales, and other Nursing Directors to ensure appropriateness of resident placement, continuity Of care, coordination Of services, and implementation Of organizational best. • Comprehensive continuum of care, including Independent Living, Assisted Living, Memory Care, Skilled Nursing, Short-Term Rehabilitation, Hospice, Home Health, PACE, and Community-Based Services.
