Develop, implement, review, and update individualized person-centered support and care plans, including the Coordinated Service and Support Plan (CSSP), Self-Management Assessment (SMA), Individual Abuse Prevention Plan (IAPP), and physician-directed plans of care, as applicable. Coordinate medical and psychiatric care, including attending appointments as required, monitoring treatment outcomes, and communicating changes in client status, needs, or response to treatment to the RN Case Manager and relevant team members.
