Job Title: RN – Case Manager (Acute Care) Location: Eureka, CA Type: Contract Duration: 13 Weeks Shift: Day Shift | Hours: 8:00 AM – 4:30 PM | Schedule: 5 days/week | Hours/Week: 40 Pay Rate: Local Pay: $60/hr (Based on experience) Travel Pay: $66/hr including stipend (Based on experience)Position Overview: Navitas Healthcare, LLC is seeking a Registered Nurse – Case Manager for an exciting Travel or Local job in Eureka, CA.Key Responsibilities<ul><li>Perform case management duties for acute care patients, including assessment, planning, and coordination of care.</li><li>Develop individualized care plans in collaboration with physicians, nurses, and interdisciplinary teams.</li><li>Facilitate patient transitions, including discharge planning, referrals, and follow-up care coordination.</li><li>Communicate with patients, families, and care providers regarding treatment plans and care needs.</li><li>Ensure compliance with facility policies, procedures, and regulatory standards.</li><li>Participate in unit assignments and float between units as needed to support patient care.</li><li>Maintain accurate and timely documentation in the Epic electronic medical record system.</li><li>Assist patients with resources and services, including housing support for travelers if required.</li></ul>Required Qualifications<ul><li>Active Registered Nurse (RN) license in California.</li><li>Minimum of 2 years of acute care nursing experience (first-time case managers may be considered with clinical approval).</li><li>Experience in case management or care coordination preferred.</li><li>Strong communication, organizational, and critical thinking skills.</li><li>Ability to work every other weekend as scheduled.</li><li>Proficiency with electronic medical records, preferably Epic.</li><li>Ability to float between units based on staffing needs.</li><li>Local candidates and travelers are welcome.</li></ul>For more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.About Navitas Healthcare, LLC certified WBENC and one of the fastest-growing healthcare staffing firms in the US providing Medical, Clinical and Non-Clinical services to numerous hospitals.
