Current New Jersey Nurse Practitioner or Physician Assistant license• National board certification as an NP or PA• Active DEA license and CDS registration• Minimum of 5 years of clinical experience in palliative care, hospice, geriatrics, oncology, family medicine, internal medicine, or complex chronic disease management• Strong leadership, communication, and interpersonal skills• Ability to work independently and effectively within interdisciplinary care teams. • Conduct comprehensive palliative care assessments and follow-up visits• Manage patients with advanced illnesses, chronic disease burden, and complex symptom needs• Facilitate advance care planning and serious illness conversations with patients and families• Coordinate care with primary care providers, specialists, hospitals, hospice agencies, and community resources• Ensure timely and accurate clinical documentation.
