Address: 5818 Centre St, Melrose, FL 32666 Our Social Worker responsibilities: · Assess, identify and coordinate plans of care and available resources to assist patient and caregivers · Advocate for patient care choices with circle of care providers · Coordinate referrals to community vendors, resources, and education · Perform advanced care planning (advanced directive) discussions and education · Counsel and support within scope of practice, with regards to social/emotional needs of patients/families/caregiver · Perform MMSE's, MOCA, GDS, clock tests and other diagnostic screenings as ordered · Initiate and lead family and patient conferences as appropriate · Work in conjunction with community resources to further develop resources/programs to meet the social/emotional needs of community elders · Maintain accurate, up-to-date documentation in the electronic medical record and departmental records · Identify, create, and presents in-services and training for staff · Participate in community outreach and education programs.
