Key Responsibilities • Conduct comprehensive nursing assessments and develop individualized care plans for home-bound patients • Administer medications, wound care, IV therapy, and other skilled nursing interventions in the patient's home • Monitor and evaluate patient progress, adjusting care plans in collaboration with physicians and the interdisciplinary team • Educate patients and family members on disease management, medication regimens, and safe care techniques • Document clinical findings, interventions, and patient responses accurately in the electronic health record • Coordinate with physical therapists, occupational therapists, social workers, and other care team members to ensure continuity of care • Identify changes in patient condition and initiate appropriate interventions or escalate to the supervising physician as needed ✅ Requirements • Active VT or Compact RN license • BLS/BCLS certification • 1+ years of Home Health experience • Proficiency in electronic health record (EHR) documentation • Strong ability to work independently, manage a patient caseload, and prioritize care in a home-based setting Pay Pay range: $2,376 to $2,466 per week. Travel Home Health RN package: up to $2,466/week (36 hours) = $37/hour taxable + up to $1,134/week in travel reimbursements if eligible under IRS tax-home and expense rules.
