The Inpatient Rehab, Care Manager MSW is responsible for providing a full range of social work services including psychosocial assessment, treatment planning, therapeutic interventions, crisis intervention, resource referral, short term counseling, grief support, advance care planning, facilitation of family meetings in collaboration with interdisciplinary team. Our not-for-profit network also provides a full spectrum of care with leading-edge diagnostics and treatment, outpatient health centers, physician groups and clinics, numerous outreach programs, and hospice and home care.
