Provider will lead and work collaboratively with a team, including nurses, medical assistants, case and care managers, behavioral health clinicians, pharmacist clinicians, and clerical staff to provide care to patients. Support from Patient Centered Medical Home team consisting of Clinical Pharmacists, Behavioral Health Clinicians, Care Managers, Nurse Practitioners & Physician Assistants along with Registered Nurses & Medical Assistants.
