Facilitate Care Plan discussion for the Interdisciplinary Team by engaging the team in conversation and documentation needed to improve or maintain the participant’s functional abilities at the highest practicable level (including auditing EMR, having team discussions in Morning Meeting, Care Planning, mini teams, etc. to ensure documentation, diagnosis, etc. Work collaboratively with all new InnovAge teams to encourage timely, accurate and quality assessments and care plans – This position is responsible for timeliness of care plan audit documentation and completion – specifically scheduling, monitoring for timeliness, compliance, etc.
