Functions as a member of a multi-disciplinary team which would include Clinic Nurse, Clinic Director, Clinic Supervisor, Individual, Legal Guardians and Residential staff if identified, and works within accordance of the individual’s needs and treatment plans. The focus is on maximizing the potential of the individuals in all facets of their life, working with individuals, families and caregivers to support individual needs, and providing appropriate medication management to individuals.
